Catchment area

Before filling out a registration form, check you are in our catchment area. Patients are accepted from most of North Cardiff i.e Llandaff North, Whitchurch, Gabalfa, Rhiwbina, Radyr, Tongwynlais, Taffs Well and Morganstown.

To register with the surgery:

Each patient must complete a 'Family Doctor Services Registration (GMS1w)' and also a 'New Patient Health Questionnaire'

To complete your registration, we will need a copy of any repeat medication you are currently taking. You should be able to obtain this from your previous GP if you do not already have a copy.

Temporary Registration

If you are ill when you are away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice.

We only accept temporary patients that are temporarily staying within our practice boundary and cannot commute to their current practice. 

We will temporarily register apatient at the point of them requiring our service. We do not proactively register patients on a temporary basis. 

 

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.

What we will do for you

  • We will treat all patients with courtesy and respect, irrespective of their race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition. 
  • We will respect our patients' privacy and confidentiality at all times, ensuring our premises are welcoming, easy for patients to find their way around and appropriate for the needs of users, including the disabled. 
  • All members of the primary care team are dedicated to providing a health service which meets our patient’s needs.

What you can do for us

  • Be courteous and polite to staff at all times.
  • Patients will be removed from our practice list if they are violent or use aggressive, rude or threatening behaviour to staff and our patients.
  • Endeavour to attend appointments at the Practice at the correct time.
